This problem has been attributed to poor weld penetration and incorrect welding procedures used during adapter installation.
To prevent possible cracking of the base cutting edge, the correct welding procedure for installation of the adapters must be used. The Special Instruction gives complete weld preparation, preheat and welding procedures for the above mentioned adapters.
