An accident can often be avoided by recognizing potentially hazardous situations before an accident occurs. A person must be alert to potential hazards. Excessive clearance between lock plate and pivot blockThe mechanical coupler has a lock plate that is mounted onto the hexagon drive of the coupler. The lock plate is designed to sit against the top of the cast pivot block. The lock plate can drift upward on the hexagonal shaft. The lock plate may not adequately contact the top of the cast pivot block. This will allow the lock plate and the hexagonal shaft to rotate. The lock plate can be modified by using the following procedure.

Correct example
Remove the lock plate (B) .
Obtain a 4B-4285 Washer .
Weld the washer to the lock plate. Use two welds (C) . Refer to illustration 5.
Test the modified lock plate on the hexagonal shaft in order to verify that the lock plate and the shaft cannot rotate.

(D) Wood block 76 mm (3 inch)
Fully retract the hook into the coupler frame.
Install a 76 mm (3 inch) square block of wood into the moving end of the coupler.
Extend the hook in order to grip the block of wood.Note: This action will duplicate the result of installing the coupler on a bucket.
Verify that the lock plate and the hexagonal shaft is not able to turn.Note: It may be necessary to install a second washer (A) in order to complete the repair.
Apply paint to the modified lock plate.
